Free 3 Month Unlimited Membership!

Life Shop is a registered non-profit in the state of Utah. We have had someone generously donate 10 three month memberships to Life Shop that will be good for the months of October, November, and December of 2023.  We would like to give these away to teens who will benefit most from them!

Please fill out this application and submit it to us no later than September 24th at midnight.  As a part of this application we are asking you to get us one letter of recommendation, so be sure to get started soon.  We will announce the 10 winners at our FREE Bingo Night on September 30.  You DO NOT need to be present to win, but we would love to have you there.

Please answer the following questions to the best of your ability.  Do not worry too much about your writing ability.  We mostly want to get to know you and how LIfe Shop would benefit you.

Please ask someone to submit a letter of recommendation on your behalf.  It can be a teacher, a coach, a church leader, a friend's parent, a boss, or anyone that is NOT a family member that knows you and would recommend that you receive a 3 month unlimited membership.  Send them this link, so they can recommend you to receive this 3 month unlimited membership.  Make sure to give them enough time to get it done, and don't forget to follow up with them, and thank them for doing it.
